Requesting Proof of Enrollment

Proof of Enrollment letters are provided upon request, twice during your program - one letter may be provided six (6) months from the enrollment date & again upon completion of the program. This letter will include: 

  • Participant’s First & Last Name
  • Participant’s DOB 
  • Participant’s Date of Enrollment in Ciba Health Program
  • Participant Activity Completion since Enrollment (at 6 month mark)
  • Participant Completion of Program - if participant remains actively engaged throughout the entirety of the program

In order to obtain a Proof of Enrollment Letter for this program, Participants must be actively engaged in their assigned cohort.

Active engagement includes:

  • Logging in to your Ciba Health dashboard via the app or web browser weekly
  • Participating in your group chat weekly at a minimum
  • Attending live sessions when able or watching the recorded session back within the same module timeframe
  • Completing at least 4 activities per module
  • Entering weekly activity minutes & weights on the app or dashboard
    • If there are connectivity issues with the scale, please contact support@cibahealth.com for assistance in a timely manner so we can help ensure you stay on track and engaged. 

 

We will not be issuing Proof of Enrollment  letters for individuals who are not actively participating.
